Fee Adjustment Form
Level Up Hoopers Basketball believes in the “no child left behind” clause. We do not allow finances to be the reason a child cannot participate in our programs. If you need assistance with the program costs for any reason (income, large household, special circumstance, etc.) please complete the Camp Fee Adjustment form.
Our Level Up Hoopers Foundation Adjustment Fee Program allows camps, clinics, and league fees to be adjusted based on parents/guardians affordability or any scholarship questions about cost adjustment.
